St. Ann Rehabilitation & Nursing Center provides high-quality, short-term skilled nursing and rehabilitation for individuals transitioning from a hospital stay due to surgery, illness or injury.

Our program includes a wide range of physical, occupational and speech-language therapies designed to enable a quick, safe and effective return home with maximum independence, full functional ability and reduced risk for setbacks.

Individualized Treatment to Fit Your Unique Needs

Planning your short-term rehabilitation visit at St. Ann begins before you even arrive. Our staff works closely with case managers and providers throughout the transition, from hospital discharge to admittance at St. Ann, to make it as easy on you as possible.

Once you arrive and get settled in, our highly-trained team of physicians, nurses, rehabilitation therapists, dieticians, social workers and nursing assistants will sit down with you to gain an understanding of your unique needs, goals and expectations. From there, we’ll develop a personalized plan of care customized to best support you – and make your time with us as productive as possible.

  • Clearly explain the process from admission to discharge
  • Help you understand your payment options, including Medicare and commercial health insurance coverage
  • Put together your own rehab team that works in coordination with your surgeon or primary care physician
  • Give you the highest level of privacy possible
  • Be available when you need us to answer questions and address concerns

Skilled Nursing and Rehabilitation Services are often provided after:

  • General or cardiac surgery
  • Joint replacement and orthopedic surgery
  • Cardiac events and interventions (such as stent insertions or treatment for congestive heart failure)
  • Strokes
  • Respiratory Issues

We offer complex nursing care for a wide range of chronic and acute medical conditions.

St. Ann’s rehabilitation services are provided by licensed Physical, Occupational and Speech-Language Therapists. In addition, our team includes Certified Occupational Therapy Assistants (COTAs) and Physical Therapy Assistants (PTAs).

Having a complete therapy team allows patients to benefit from an integrated approach to therapy. Consistent, repetitive therapy is important for an effective and timely recovery. Rehabilitation services in a skilled nursing facility provide the opportunity to receive therapy more than once a day and at times that are optimal for you. St. Ann offers that invaluable flexibility, in addition to the highest level of services.

Physical Therapy is provided by licensed Physical Therapists (PTs) who evaluate each patient and design a custom activity and exercise plan that builds strength and improves the range of motion for limbs and joints. PTs help reduce pain, restore functional ability and prevent disability.

Occupational Therapy is provided by licensed Occupational Therapists (OTs) who perform evaluations and help customize therapy that improves the patient’s abilities to perform daily tasks, ranging from self-care to recreational or occupational activities. In addition, OTs design treatment plans and assist patients in improving function, visual, cognitive and general coordination skills. The ultimate goal of occupational therapy is to facilitate a patient’s ability to perform with the highest degree of independence possible.

Speech-Language Therapy is provided by licensed Speech-Language Pathologists (SLPs) who address communication, cognition and dysphagia (swallowing) deficits that may be due to injury, illness or chronic decline. SLPs provide therapeutic interventions to improve receptive and expressive communication disorders, cognitive-communication disorders and speech impairments due to various types of medical conditions and disorders. SLPs also establish communication systems for patients with hearing impairments, and for those requiring alternative communication systems. Additionally, SLPs play a primary role in the evaluation and treatment of adults with swallowing disorders.

St. Ann’s short-term rehabilitation care program is housed in its own dedicated wing, with 20 private rooms. Residents receive 24/7 nursing care and support.

We do everything we can to prepare you for a safe return home, which means simulating your normal lifestyle, and creating an environment that mimics your everyday life and routines as much as possible. Your dedicated team of physical, occupational and speech therapists is continuously focused on your own goals and providing services at times that are best for you and your schedule.

Our two rehabilitation gyms keep you determined and focused as you work towards your goals. And our community area – with plenty of natural light and a nautical theme that encapsulates the essence of the Seacoast – is the perfect place for downtime, whether it’s reading or socializing with other residents.

We want St. Ann to feel like it’s your own home. You may bring almost anything to make you feel comfortable, including a favorite pillow, photos of your family, your clothing, electronics or personal care items. There is a kitchen on the floor for you to enjoy an array of meal options prepared by our culinary chef. Your room comes equipped with Wi-Fi, phone and cable television, so you can continually stay connected with loved ones, the world around you or your favorite television program. Visitors – whether family or friends – are always welcomed.

With today’s shorter hospital stays, many individuals require additional therapy and skilled nursing services for a short time following discharge from the hospital. Often, the necessary services aren’t practical to be provided at home. St. Ann provides short-term skilled nursing care and rehabilitation therapy that can help enable a safe, effective and quick return to independence

If you have Medicare coverage, you may qualify for the Medicare Skilled Nursing benefit. If you have commercial health insurance through an employer, you may also be eligible for skilled nursing services. Coverage should be verified through your insurance provider or the hospital’s discharge planner.
