Long-Term Care

24/7 Support, A High Quality of Life

St. Ann Rehabilitation & Nursing Center provides long-term care for individuals who require 24/7 nursing care and support, as well as an enriching environment where they can thrive and live their lives to the fullest every day.

We’re a close-knit community that is continually looking out for your needs and best interests through our person-centered model of care. We work with you so you can live life on your own terms while finding the optimal balance between physical, emotional, social and spiritual wellness. Most importantly, we are a place you can proudly call home, find joy in all that you do and become a productive member of our community.

Is Long-Term Care Right For You?

The thought of leaving your home – a place full of cherished memories and warm feelings – can be a difficult and emotional process.

You might worry you’ll lose your independence, miss your familiar surroundings and no longer be able to partake in the simple things in life that matter most. For your family, there might be anxiety about how you will respond to a new environment and interact with those around you.

But in many cases, long-term care is the best way to meet your increasing daily needs – across all facets of life. It’s something we’re confident you’ll realize here at St. Ann.

Today’s skilled nursing facilities offer more services and amenities, and have more favorable levels of nurse staffing. They can also be a better value with a single, all-inclusive cost per day. There are more resources and flexibility to give you the true feeling of being home.

Our team is ready to explore those choices with you and your family when home care alternatives have been exhausted.

Why St. Ann?

We’ve been serving the long-term care needs of individuals across the Seacoast Region and beyond since 1958. Our focus on person-centered care not only means your transition will be as stress-free as possible, it means your time here – and the support you receive – is solely based on your own needs, wishes and preferences.

When you join our community, we first sit down with you and your family members to listen and ask questions. We get to know everything about you, including:

  • Your medical and life history
  • Your hobbies
  • What makes you most happy
  • Your routines
  • Your limitations
  • What makes you feel most safe and comfortable
  • What keeps you motivated
  • Your goals and expectations
  • Your nutrition and dining preferences
  • How we can best support you… and so much more.

Each resident is different, and that’s why we come up with an individualized plan of care centered on these areas. This is essential in creating an environment where you not only engage more in life – in both new and old ways  – but one where you experience a high quality of life. The way we see it, you direct your own care.

Your safety here at St. Ann is paramount too, with 24/7 care and support. The nursing staff is never more than a few feet away and our electronic call boards ensure we always respond to your needs in the most timely and efficient manner.

It is no surprise that many of our residents find that living here is a preferred alternative to living alone or with family members.

Please speak with an admissions coordinator for more detailed and specific information.

When you come through the doors of St. Ann, you not only enter a building that has proudly served the needs of thousands of residents over the past 60 years, you become part of a vibrant community.

Step into the lobby area and you’ll notice how quaint a community we are. As you walk down the front hall and pass the activity room and dining area, you will see there is always something to do – whether it’s residents taking in new hobbies, reading a new mystery book, listening to music or enjoying plenty of social time (and laughs) with each other.

All of our 54 resident rooms (mostly private) are spaced out across several wings, giving you the feeling of a neighborhood community. Rooms are personalized in a way that makes it feel like home. You’re welcome to bring almost anything, whether it’s your favorite pillow, photos of your family, your clothing, electronics, personal care items or even your own furniture. Your room comes equipped with Wi-Fi, phone and cable television, so you can continually stay connected with loved ones, the world around you or simply enjoy your favorite television program. Visitors are always welcome.

Our executive chef is always at the ready with various culinary options and flexible meal schedules. You can enjoy fresh food in our beautiful restaurant-style dining room or in the privacy of your own room. Guests are welcome to visit our gift shop for small food items, drinks and other accessories.

There is never a dull moment, either, for those who love to stay busy. Regular events include games, entertainment, movies, shopping trips and plenty of music. You can always balance that with downtime and rest as needed. The outdoor terrace is the perfect place to reflect or spend precious time with family.

Your safety, health and well-being form the foundation of the high-quality healthcare at St. Ann.

We are proud of our resident and family satisfaction levels, which are both well above national averages. 100% of our residents would recommend St. Ann to others.
